Due to one of our NP team mates reducing hours to PRN , we're looking for an experienced APC to bring into the practice to provide primary care access to Internal Medicine - Adult / Geriatric patients. This provider will be managing his/her own panel as well as access for acute issues. You will have the ability to take over the panel of the departing NP and grow your volume through new patient referrals. Looking for a collaborative, true team-player, with a positive attitude, ability to work well with others, a love of patients and focus on quality outcomes.

Experience Preferred: number of years is less important than where the APC has been practicing, and to what they have been exposed. Must have at least 1 to 2 years of independent, ambulatory practice in a high volume, high acuity environment.
  • Full time - Monday through Friday - 8:00-5:00
  • 36 patient contact hours - 4 hours of Admin time weekly
  • 100% outpatient practice
  • Call is every 3rd week - fewer than 10 calls per week - No Rx refills for controlled substances
  • Average 16-18 patients per day - all patients age 18+
  • Telemedicine and E-visits conducted through My Chart and
  • Must have practice experience with basic primary care skills and geriatric medicine
  • Ability to manage multiple chronic conditions
  • Clinical Support staff includes 1:1 Provider/CMA, LPN's and one RN
  • CDE, Xray, Dexa scan and phlebotomy in practice
