Experienced Senior Services - Geriatric Medicine APC-NP

The Nurse Practitioner who joins our team of compassionate providers Must Love working with our elderly patients. We are looking for a highly-skilled, team player with a strong background as a hospitalist in an acute care setting, or a seasoned geriatric care provider. Our Senior Care Services team provides medical care in our 10 clinics around Richmond; including Long term care, Skilled Nursing Facilities and Nursing Homes. You must be flexible, able to work independently, have reliable transportation and confidence in your clinical expertise. A friendly, professional, collegial Nurse Practitioner is required for this position.
  • Position is Salaried, Full-time Employee (40 Hours), Monday - Friday 8a-4p
  • Facility location is on Parham Road, Richmond
  • NP will provide primary care and round on patients in the facility, review Rx requests and lab results, attend facility meetings, meet with families, etc.
  • Approx 15-20 patients per day
  • NP will provide educational support to LTC staff as needed and requested by facility leadership
  • Strong team and support - 4 MD's and 4 APC's
  • Call Schedule is shared by providers in the group - typically 4 days per month avg and one weekend call every other month.
