Home Based Primary Care Nurse Practitioner

The Nurse Practitioner who joins our team of compassionate providers Must Love working with complex, high acuity patients who are in their own home setting. We are looking for a highly skilled team player with a strong background in primary care practice - 3 to 5 years preferred - or a seasoned geriatric care provider. You must be flexible, able to work independently, have reliable transportation and confidence in your clinical expertise. A friendly, warm, professional Nurse Practitioner is required for this position. You'll work as part of a patient care team which includes an MD, RN, social worker and patient service representative.
  • Position is full-time (40 Hours), Monday - Friday 8a-4p
  • 2 call weeks per year - covering Home Based Primary Care, Hospice and Palliative services
  • The program serves patients in the Richmond area
  • NP will provide primary care to approx. 4 patients per day
  • Patients are all adult and 95% are on Medicare
  • Strong team support
  • Must be proficient managing multiple chronic conditions and high acuity patients
  • We're looking for a really good team member with a positive attitude and focus on quality patient care and outcomes. We support each other here, often switching call days to help when needed, or pitching in during FMLA or same-day call out.
  • Good business skills - close encounters same day - return patient calls and emails - good communication and maturity
